Thus, if the donors do not meet the recipient couple, the receiver can bring the donor for the treatment.

Tests that are being evaluated from the donors:

The donors are examined with respect to infectious diseases (HIV, Hepatitis B and C, Cytomegalovirus, Syphilis, Chlamdydia) and genetic diseases (Karyotype analysis, Cystic Fibrosis analysis,  Maple Syrup Urine Disease (Phenylketonuria) and screened for Thalassemia minor or major, Haemophilia A and B, Familial Mediterranean Fever and Sickle cell anemia (applicable for particular ethnic origins), whose blood types and Rh conditions are determined.

Criteria For Egg Recipients:

Tests that are being evaluated from recipients:

Both partners are screened to determine their suitability for the donation process. They are evaluated and counselled in order to determine that they fully understand the nature of the process. The physical screening of the female partner includes examination by a gyneocologist and several tests such as:

  • Blood tests including, HbsAg, Anti-HCV, Anti-HBC total, HIV, VDRL.
  • Blood type and Rh factor,
  • TSH, T3, T4,
  • Glucose test,
  • CBC (Complete Blood Count)
  • Urea, SGOT, SGPT,
  • FSH, LH, E2 and PRL, AMH

The male partner is screened also for blood tests and will also have  to make the following analysis:

  • Semen analysis
  • Blood type and Rh factor
  • Blood tests including, HbsAg, Anti-HCV, Anti-HBC total, HIV, VDRL.

Step 2:

Selection of the Suitable Donor:

In this step we ask from the recipient to send us some details about themselves such as  their characteristics, nationality, photographs and blood types to check the most suitable donor for them. The team that they are selecting the most suitable donor for the recipient takes the following issues under consideration to decide:

  • Nationality
  • Skin color
  • Eye color
  • Height
  • Weight
  • Hair
  • Education

After recieving the test results that we asked from the recipient we manage a treatment schedule for each patient that it should be followed from a doctor in their own countries, its up to the patient if they want to be followed by their own doctor or the doctors that we could advice them from our own lists selected in  the nearest place or even the same city could be given as a choice. Duration Of The Preparation For The Egg Donor:

An egg retrieval procedure comprises both the Egg Donor’s Cycle and the Recipient’s Cycle. Birth control pills are administered during the first few weeks of the egg donation process to synchronize the donor’s cycle with the recipient’s, followed by a series of injections. These injections may be self-administered on a daily basis for a period of one to three weeks. Next, follicle-stimulating hormones (FSH) are given to the donor to stimulate egg production and increases the number of mature eggs produced by the ovaries. Throughout the cycle the donor is monitored often by a physician using blood tests and ultrasound exams to determine the donor’s reaction to the hormones and the progress of follicle growth. When a “fresh cycle” is followed, the success rate with donor eggs goes up to approximately 80%. With egg donation, women who are past their reproductive years or menopause can become pregnant.

The receiver is given estrojen on ascending doses as soon as the donor starts with the egg augmentation treatment. Estrogen makes the intrauterine layer thicker so that the retention of the transferred embryos is ensured. On the same day the eggs of the donor are collected, her partner’s sperms are sampled and the fertilization process gets started. (ICSI process which is the fertilization procedure routinely  used in our laboratory). In cases of unavailability of sperm in the semen, the sperm is obtained by surgical techniques and extracted from the testes or scrotum. In other cases, couples may choose to use donor sperm. The receiving patient usually begins to use progesterone via vaginal route on the same day as egg collection process.

The fertilized embryos are placed in a fresh cleavage medium and left to develop for several days before the transfer. The transfer is generally done on day 3 when embryos are normally developed to the 6 or 8 cell stage. In some cases, depending on the embryo quality, they are allowed to develop until day 5 up to the blastocyst stage that contain more then 100 cells. The administration of progsterone and estrogen is maintained after the process of transfer. 12 days following the transfer process, the pregnancy result is found out by analysing the ßHCG in the blood.

Note:It is often the case in IVF with egg donation that more embryos are produced than can be used in one cycle. When this happens, those embryos can be frozen and preserved for future use.

Also note that for both menstruating women and menopausal women, the treatment stages are the same. Both must take medication prior to the embryo transfers.

Embryo Transfer :

A few days after the donor eggs are fertilized by your partner’s  sperm, you will come to our laboratory for the embryo transfer. Transfer is a simple and painless technique but some patients require anesthesia to relax their muscles. On the same day the embryologist will give you information about the quality of embryos to be transferred. The procedure is very similar to that of intrauterine insemination (IUI).

You have to drink approximately 1 lt of water before the transfer, as a full bladder allows the Doctor to identify the uterus using a trans-abdominal ultrasound more easily, it straightens out the cervix and allows the transfer to go more smoothly.
